Problem Description:
In the Task Manager, new application windows lose their correct application icon and instead display a generic Wayland icon.
This issue occurs across all applications, including KDE system applications (e.g. Security Center / System Settings), not just third-party or Electron apps.
The issue is reproducible whenever an application creates additional windows or toplevel surfaces.
Steps to reproduce:

Open any application (e.g. Dolphin, Firefox, System Settings)

Trigger a new window within the same application

Observe task manager behavior:

First window appears with correct icon

New window briefly shows correct icon
-
Then switches to a generic Wayland icon

In some cases, task entries duplicate or replace each other
